Tips for successful DTB visit:

Complete and submit this form in advance of your visit
Bring valid Government issued form of photo id on the day of your visit. You will not be able to check into our facility without it
Display your DTB Visitor badge while you are in the facility and remain with your assigned DTB Escort
Turn in you badge when you leave the facility for the day/night to either the security guard or the receptionist
Security policies prohibit video and photo recording devices from being brought into the facility, so please do not bring them with you
