Dayton T. Brown, Inc. welcomes your visit to our facility.
The safety and security of our employees, customers and their programs necessitates that all personnel visiting our facility, whether customer or supplier, notify us of their visit in advance by completing and submitting the DTB Visitor Information form.
Please select the form that best describes you and your visit. After selecting the appropriate form, fill in the information requested and submit the form online or fax a hard copy of the form to

If your visit is related to a classified program, you must contact the DTB Security Manager for classified visit instructions.

- I am a United States Citizen or a Permanent Resident of the United States and my program or purpose for visiting is short term (30 days or less).
- I am a US Citizen or a Permanent Resident and my program/purpose for being here will be long term (31 days or longer).
- I am NOT a United States Citizen or Permanent Resident.
Please remember to bring valid, government issued identification with you. Please do not bring photographic equipment with you unless advance arrangements have been made and authorization has been granted by the DTB Security Manager. Cell Phones are permitted in the facility, but use of the camera feature is prohibited.
Our front lobby is currently under construction for remodeling. Visitors will enter our facility through the west side entrance of the laboratory building, through the door adjacent to the guard booth.
As you enter, there is an intercom system directly in front of you. Please touch the button as indicated and you will be provided with further instructions.
